This document outlines the types of personal information we receive and collect when you use ALZCARE’S services, as well as some of the steps we take to safeguard information. We hope this will help you make an informed decision about sharing personal information with us. Personal Information and Other Data We Collect 1. ALZCARE collects information (personal, regarding the prospective resident, regarding your care facility, etc.) when you register for an ALZCARE service or otherwise voluntarily provide such information, whether using the website, sending an email or calling us directly. 2. With licensed care facilities and health care professionals ALZCARE may combine information collected from you with information from third parties (for example, state licensing agencies, professional associations, etc.) to provide the most accurate and comprehensive service for our clients. 3. ALZCARE may use cookies and other technologies to enhance to improve the quality of our services (such as in allowing you to save work on a form in order to continue the work later). 4. ALZCARE’S servers automatically record information when you visit our website or use some of our services, including the URL, IP address, browser type and language, and the date and time of your request. Uses 5. We may use personal information to provide the services you've requested, including services providing you with a personalized list of care homes. 6. We may use email addresses to deliver newsletters or other information. 7. We may also use personal information for auditing, research and analysis to operate and improve ALZCARE technologies and services. 8. For care homes and health care professionals we share pertinent information with families seeking a care facility or seeking professional assistance. 9. For persons seeking a care facility, we share personal contact information and detailed prospective resident personal and care needs information only with care facilities who have agreed to protect and never share that information with persons or entities outside of the care home, other than with official state agencies engaged in the supervision of that care home as required by state law. 10. We may also share information with third parties in limited circumstances, including when complying with legal process, preventing fraud or imminent harm, and ensuring the security of our network and services. 11. We never share or sell personal information, including email address, to third parties for marketing purposes beyond the direct care home placement process that the client is requesting. Your Choices 12. Any site visitor may refuse cookies in your browser by the settings you choose in your browser. We do not promise that all of our site features or services will function properly in this case. 13. Any person receiving any email (newsletter or any other content) may request to have their address removed from the ALZCARE mailing list. 14. Clients seeking care home referrals may direct ALZCARE not to share any identifying personal information of the responsible party and/or the prospective resident with any care homes (names, phone numbers, etc.). In such cases, we will provide care homes with care need specifics and the information that the responsible party will contact them with an identifier (e.g., ALZCARE JIM) to help them match the caller with the care needs profile. Clients are hereby advised that this approach will likely slow the process of obtaining placement. 15. We make good faith efforts to provide you access to your personal information upon request and to let you correct such data if it is inaccurate and delete it, when reasonably possible.
