The purpose of ALZCARE is clearto help our clients find
the very best Assisted Living Care Home options for their
loved one who has a dementing illness such as Alzheimers
disease (see Who We Are).
In order to accomplish this ALZCARE is constantly gathering
detailed information about Assisted Living Care Homes across
the United States.
When you contact ALZCARE (by Phone,
Online Form, etc.) we begin collecting
information about your situation. The information we ask of
you serves numerous purposes, but four of the most important
are:
Confirming that you are the (or one of the) Legally
Responsible person (or his or her designate) who we need
to speak with in this process;
Clarifying and prioritizing with you the key criteria for
the placement (see Priorities);
and
Creating a Prospective Resident Profile (PR Profile) which
clearly explains the care needs (ADLs
and dementia specific) of the prospective resident. This
Profile closely mirrors the Care Home Profiles (CH Profile)
we create for each Assisted Living Care Home, thus allowing
us to find the best matches for both current and future care.
Each placement process that we assist with is unique and will
diverge in minor or significant ways from our general procedures.
But the basic process is to use the Prospective Resident Profile
to identify the Assisted Living Care Homes that best match
your needs. We then communicate with those care homes to confirm
their services, interest, space availability and so on as
appropriate.
This process can take as little as a few hours and as long
as a few weeks, depending on many factors. But we always strive
to meet the timeframe that you have identified in your Profile.
Once the best matches have been identified and confirmed we
supply them with your contact information (unless you instruct
otherwise) and you with their contact information. Over the
coming days and weeks (depending on your urgency and schedule)
we will work with you as you set appointments and begin reviewing
and evaluating the different homes. Along with their contact
information we also supply you with guidelines and checklists
to guide you in the evaluation process.
It is usually, but not always, the case that our clients are
satisfied with the options we provide. If for some reason
we did not manage to provide you with care home options that
you found acceptable we work with you to find out where the
problems or discrepancies were and to correct them in the
next round of referrals.
Once you do find the best match for your loved one we help
you further with guidance in making the actual move and transition.
